Antecedentes: Ensaios clínicos randomizados recentes demonstraram consistentemente a segurança e a eficácia potencial da terapia MSC para pacientes com insuficiência cardíaca. Este estudo investiga o potencial promissor das células-tronco mesenquimais, oferecendo um farol de esperança para o futuro do tratamento da insuficiência cardíaca com fração de ejeção reduzida (HFrEF).

Métodos: Seguimos as diretrizes de itens de relatório preferenciais para revisões sistemáticas e metanálises para esta revisão sistemática e metanálise. Pesquisamos quatro bancos de dados e registros de ensaios clínicos randomizados, incluindo PubMed, EBSCO, clinicaltrials.gov, ICTRP e outros sites relevantes. Em seguida, selecionamos treze ensaios clínicos randomizados com 1184 participantes com base em nossos critérios de inclusão/exclusão predefinidos. Dois avaliadores independentes extraíram os dados e realizaram uma avaliação de qualidade. Os dados foram então plotados para vários resultados, incluindo morte, hospitalização, eventos cardíacos adversos graves, parâmetros da função da bomba e distância de caminhada de 6 minutos.

Essa descoberta reconfortante ressalta a confiabilidade da terapia baseada em MSC, independentemente de sua origem. No entanto, AutoMSCS mostrou uma tendência de maiores benefícios de proteção. A exclusão de estudos usando células derivadas do tecido adiposo resultou em uma segurança ainda melhor e uma melhora significativa na LVEF para o tratamento com alloMSCS.}

Conclusão: Nossos resultados sugerem que ^ALLOMSCS estão em pé de igualdade com ^AutoMSCS na melhoria dos resultados funcionais em pacientes com insuficiência cardíaca. Isso ressalta a necessidade de futuras investigações em uma coorte maior de pacientes, enfatizando a urgência e a importância de novas pesquisas para compreender completamente o potencial das MSCs no tratamento da insuficiência cardíaca.
